School of Worship – Spring Term 2026

By registering for and participating in the School of Worship (SOW), I acknowledge, understand, and agree to the following terms and conditions:

1. Participation & Conduct

I understand that the School of Worship is a faith-based training program conducted by Alpha & Omega International Christian Fellowship. I agree to conduct myself in a respectful, orderly, and Christ-honoring manner at all times and to follow all instructions given by church leadership, instructors, and volunteers.

I acknowledge that:

• Attendance and punctuality are expected.

• Participants are not permitted to leave the church/facility premises during sessions unless explicitly approved by authorized leadership.

• Disruptive behavior may result in dismissal from the program without refund.

2. Photography, Video & Media Consent

I hereby grant Alpha & Omega International Christian Fellowship and the School of Worship the irrevocable right and permission to take, use, reproduce, publish, and distribute photographs, audio recordings, and video recordings of me (or my child) taken during School of Worship sessions for ministry, promotional, educational, archival, and media purposes, in any format, without compensation.

I understand that:

• Participants are NOT permitted to take photos, videos, or audio recordings during sessions unless explicitly authorized by Alpha & Omega International Christian Fellowship leadership.

• Unauthorized recording or distribution of content may result in removal from the program.

3. Assumption of Risk & Release of Liability

I understand that participation in School of Worship activities may involve inherent risks, including but not limited to physical movement, equipment use, and group activities.

I hereby voluntarily assume all risks associated with participation and release, waive, discharge, and hold harmless Alpha & Omega International Christian Fellowship, its pastors, staff, volunteers, instructors, and affiliates from any and all claims, liabilities, damages, losses, or causes of action arising under state or federal law, whether known or unknown, arising from participation in the program, except where prohibited by law.

4. Medical Emergency Consent

In the event of illness, injury, or medical emergency, I hereby give permission to Alpha & Omega International Christian Fellowship and its representatives to:

• Administer basic first aid as deemed necessary

• Contact emergency medical services (EMS)

• Transfer me (or my child) to a hospital or medical facility as required by law or medical necessity

I understand that Alpha & Omega International Christian Fellowship will make reasonable efforts to contact the emergency contact listed but may act without delay if immediate medical care is required. I accept full responsibility for any medical costs incurred.

5. Compliance with Rules & Policies

I agree to comply with all safety policies, facility rules, and program guidelines established by Alpha & Omega International Christian Fellowship and the School of Worship. Failure to comply may result in dismissal from the program without refund.

6. Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the State of Texas and applicable federal laws.

7. Acknowledgment & Signature

By registering for this event online, I affirm that I have read and fully understand this agreement. If Participant is under 18 years of age, I, as the parent/legal guardian, consent to my child’s participation and agree to all terms stated above. I voluntarily agree to its terms and acknowledge that this agreement is legally binding. I will also sign this form physically in-person before the term begins.
